Community Health Programs
All programs and classes are designed specifically to create transformative, healthy, life-affirming changes in the lives of patients by promoting healing and wholeness in mind, body, and spirit.
Diabetic Self-Management and Education program educates patients about the disease and how to manage it. This program is done in groups and individual cases using Certified Community Health Workers to provide education to clients in a consistent repetitive manner with an emphasis on follow up case management. Becoming a Mom prenatal program is a bilingual prenatal curriculum designed for use with pregnant women in a supportive group setting to learn the importance about having a healthy pregnancy. These classes are also taught to educate women on the importance of being healthy before pregnancy. To register for this class, contact Genie Chargois at (409) 832-1924 ext 105."
Community Outreach program consists of monthly wellness programs that promote health literacy andeducation. Our Community Health Workers connect clients to local health and social resources, help clients navigate and apply for government programs, and provide case management services. We also provide classes that help clients build resources for a better life through the Getting Ahead program through the Bridges Out of Poverty initiative.
Health Is Hip, our children’s health initiative program, is designed to use fun and interactive games to help kids learn about nutrition, exercise, and healthy habits. This program focuses on the prevention of childhood obesity and diabetes. Approximately one-third of Americans age 2 to 19 are overweight or obese. Ubi's goal is to empower the youth of Southeast Texas to adopt healthy lifestyles in an attempt to reverse the increase in childhood obesity and the chronic conditions it causes.
CAMP 5210 helps children and their families become more fit by participating in fungames and activities that promote healthy lifestyles. Sessions provide instruction relatedto nutrition, fitness, health, and behavioralissues which parents or guardians can use to improve the overall health of their children and their families. A nurse helps families with health related questions. This program is done in collaboration with the Junior League of Beaumont and Christus Health.
Media-Smart Youth is an interactive education program for youth ages 11 to 14. The curriculum is designedto empower young people to become aware, and think critically about media's role in influencing their nutrition and physical activity choices. It also helps them to build skills that help them make informed decisions about being physically active and eating nutritious food in daily life. The youth will participate in 10 structured lessons that culminate in a Big Production.
The “R” Rules is largely based off the ideas of the Getting Ahead Class. It is designed for middle and high school ages. Through class sessions, teens learn how to identify and build resources necessary to further their goals for college, technical school, and the work force.
Reach Out and Read is an evidence-based nonprofit organization of medical providers who promote early literacy and school readiness in pediatric exam rooms nationwide by giving new books to children and advice to parents about the importance of reading aloud. Reach Out and Read builds on the unique relationship between parents and medical providers to develop critical early reading skills in children, beginning at 6 months of age. Reach Out and Read families read together more often, and their children enter kindergarten better prepared to succeed with larger vocabularies and stronger language skills.

In October 2012, our Family Medical Clinic became Legacy Community Health Services – South Park. This new partnership between Legacy and Ubi Caritas will provide access for thousands of more people as Legacy brings the resources of a much larger organization. Learn more.
In October 2011, Ubi Caritas formed an association with Family Smiles Dental Center to provide General Dentistry for Adults & Children. Family Smiles accepts Medicaid & CHIPS, cash patients w/out Insurance. Learn more.
